MEDICAL INFORMATION FOR ALL FAMILIES
If your child has a medical condition, please read the following information and complete all required steps. To support your child’s health and safety, all medical conditions require:
1. A current Medical Action Plan (colour version)

- Official action/management plan for your child’s condition, valid for the current school year
- Must include a recent student photo (passport style/school photo), all sections completed (symptoms, triggers, emergency contacts, instructions), and signature of a medical practitioner (where applicable)
2. Medication
-
- Provided on your child’s first day of the current school year
- In-date, original packaging, and clearly labelled
- Must be accompanied by an OSPS Medical Authority Form
3. Condition-Specific Information Form

- Provides staff with details about your child’s symptoms, daily support, and emergency actions
- Forms are Google Forms and must be completed for each condition – available under each condition’s tab
If your child has a new medical or learning condition, or an existing condition is no longer ongoing, please contact the Office as soon as possible. A note from a medical professional confirming this is required for us to update our records.
Submitting forms and documents:

- Submit all required plans, medications, and forms to the General Office on your child’s first day

Important:

- Most medical action plans must be completed and signed by a medical professional.
- Colour versions of all action plans are required to assist staff during medical emergencies.
- Include a recent student photo (passport style/school photo) on action plans where required.
- Medications must be in-date, clearly labelled, and provided in original packaging.
- Submit all plans, medications, and forms together on your child’s first day of the school year.
ANAPHYLAXIS INFORMATION
What to Provide:
- ASCIA Anaphylaxis Action Plan (colour version), completed and signed by medical professional
- Required medication (e.g. adrenaline auto-injector, antihistamines)
- Provided on your child’s first day of the current school year
- In-date, original packaging, and clearly labelled
Submission of these plans, medications, and forms is required under DET and school policy to ensure your child’s safety and comfort at school, and to minimise disruptions to your day. The school will always contact parents or guardians if a serious medical incident occurs. Families are encouraged to maintain ambulance cover in case of emergencies.
ASTHMA INFORMATION
What to Provide:
- Asthma Australia Action Plan (colour version), completed and signed by medical professional
- In-date reliever medication (e.g. Ventolin) with spacer and mask (if required)
- In-date, original packaging, and clearly labelled
- Provided on your child’s first day of the current school year
Submission of these plans, medications, and forms is required under DET and school policy to ensure your child’s safety and comfort at school, and to minimise disruptions to your day. The school will always contact parents or guardians if a serious medical incident occurs. Families are encouraged to maintain ambulance cover in case of emergencies.
ALLERGIES INFORMATION
What to Provide:
- ASCIA Allergy Action Plan (colour version), completed and signed by medical professional
- In-date medication required at school (e.g antihistamine)
- Provided on your child’s first day of the current school year
- In-date, original packaging, and clearly labelled

HAY FEVER INFORMATION
What to Provide:
- ASCIA Hay Fever Action Plan

Any required medication (e.g. antihistamines, nasal sprays)
Submission of these plans, medications, and forms is required under DET and school policy to ensure your child’s safety and comfort at school, and to minimise disruptions to your day. The school will always contact parents or guardians if a serious medical incident occurs. Families are encouraged to maintain ambulance cover in case of emergencies.
ECZEMA INFORMATION
What to Provide:
- ASCIA Eczema Action Plan (colour version), completed and signed by a medical professional
- Any prescribed creams or medications
- Provided on your child’s first day of the current school year
- In-date, original packaging, and clearly labelled
- OSPS Medication Authority Form
Submission of these plans, medications, and forms is required under DET and school policy to ensure your child’s safety and comfort at school, and to minimise disruptions to your day. The school will always contact parents or guardians if a serious medical incident occurs. Families are encouraged to maintain ambulance cover in case of emergencies.
OTHER/COMPLEX MEDICAL CONDITION INFORMATION
For conditions that may affect health, safety, learning, or participation (e.g., migraines, seizures, chronic pain, anxiety, ADHD, ASD, blood disorders, heart conditions, etc.):
What to Provide:

A Medical Action Plan or a letter from a treating medical professional outlining your child’s condition, care needs, and emergency management

- Provided on your child’s first day of the current school year
- In-date, original packaging, and clearly labelled

OSPS Medical Authority Form (must be completed for any students requiring medication at school)

NEURODEVELOPMENTAL, LEARNING & FUNCTIONAL SUPPORT
For any diagnoses, such as ADHD, ASD, or other learning/support needs, or support needs affecting learning, behaviour, emotional regulation, or participation at school.
What to Provide:

Relevant diagnostic/assesment documentation
- Neurodevelopmental, Learning & Functional Support Form (Google Form)

If your child takes medication at school:

Required medication in a compact weekly supply container (please only provide one week at a time)

OSPS Medical Authority Form (must be completed for any students requiring medication at school)
